What will happen if members have difficulty logging in to see their digital results?

Members will be prompted to call the Everly Health customer care team at 855-923-2678 for the following reasons:

  • If a member enters mismatching member ID, name, date of birth and/or zip code, they will not be able to proceed to their results. 
  • A member’s account will be locked after 5 login attempts. Once locked, the member can either wait 15 minutes to try again or call our customer care team for assistance. 
  • If the member enters a health plan name that is not in the database, they will not be able to proceed to the next screen. The database menu includes all health plans listed on cms.gov.
