Everly Health receives an eligibility file with member and provider information. Required DirectTrust eligibility file header fields:

Letter: In the eligibility file, the PCP mailing address is required to successfully transmit a letter to a provider. If a letter is the fallback method, it is expected PCPs will have an address included in the eligibility file records. These eligibility file headers include:
